• Out of Network:  Medicaid, Medicare, Horizon NJ Family Care, Horizon Managed Care Plans, EAP, etc. 

Out of Network Benefits: If you wish to participate in treatment using out of network benefits, payment is due at the time of each session payable by cash or credit card. I will be providing you a bill with all pertinent information for you to submit to your insurance company for out-of-network reimbursement to be paid to you directly by your insurance company. Please discuss your eligibility for out of network benefits with your insurance company to verify reimbursement procedures.

Cancellation Policy

Consistency is one of the essential components to therapy, and it is required for effective treatment. If you do not show up for your scheduled appointment time without 24 hours advance cancellation notice, you will be required to pay a $50 cancellation fee. I can further address any concerns or details in regards to rescheduling and make-up policy during the initial consultation.

Sliding Scale

I have limited availability for sliding scale treatment. I typically reserve these spots for students or current patients undergoing financial hardship. I encourage you to discuss finances if they are a concern.
